We love hearing from successful clients all across the country and are always looking for motivational stories to feature on our Web site and in our advertising. So, if you’ve reached your goal weight and want to show it off, please let us know! Just follow the steps below to tell the world your Herbal Magic success story.

1. You must complete a Client Testimonial Release form. To read and print the Client Testimonial Release form, click here.

2. You must fill out all sections of the Client Testimonial Release form, including personal vitals, weight and inches lost, and your Herbal Magic experience.

3. Have your health counsellor, or the manager at the centre you attend(ed), sign the form as the “Witness”.
4. Send the form, along with some pre-program "before" photos and some new, full body “after” photos to the address below. We accept developed photographs but prefer you send 300 resolution or higher digital photos on a CD if possible. Your photos will become the property of Herbal Magic and will not be returned.
